29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At a residential worksite located at 5423 NW Platte Purchase Dr, KC MO -- employees engaged in roofing duties using hammers, staplers, and pneumatic nail guns were not wearing eye protection. This exposed employees to struck-by hazards in that debris could get in their eyes. This was documented on or about 07/14/2021.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At a residential worksite located at 5423 NW Platte Purchase Dr, KC MO -- Employees performing roofing duties did not have any kind of fall protection system in place. Employees were exposed to fall hazards greater than 18 feet above ground level. This was documented on or about 07/14/2021.
