29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program: a) Appalachian Wood Products, Inc. - On about August 31, 2021, employees exposed to noise levels greater than 85 dBA, while conducting work operations with or around saws, molders, drills, and sanders, were not provided with annual noise/hearing conservation training. Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels : a) Appalachian Wood Products, Inc. - On or about about August 31, 2021, employees exposed to noise levels greater than 85 dBA, while conducting work with and around saws, molders, drills, and sanders, were not provided with annual audiometric testing. Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
