29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Appalachian Wood Products, Inc. 171 Appalachian Drive Clearfield, PA 16830 - On or about 04/06/2023 and at times prior thereto, employees were exposed to hazards associated with the back side of machine #995 Accu-Systems miter saw (serial # 005-06-MMTJ-3-APPPRO) in the assembly area was not guarded to protect employees from moving parts in the Assembly Area. b) Appalachian Wood Products, Inc. 171 Appalachian Drive Clearfield, PA 16830 - On or about 04/06/2023 and at times prior thereto, employees were exposed to hazards associated with a rotating coupling on machine #664 Mereen-Johnson Double End (serial #MJ1069) in the Assembly area. The coupling is not guarded to protect employees from the rotating parts. c) Appalachian Wood Products, Inc. 171 Appalachian Drive Clearfield, PA 16830 - On or about 04/06/2023 and at times prior thereto, employees were exposed to hazards associated with a rotating hub on machine #664 Mereen-Johnson Double End (serial #MJ1069) in the Assembly area. The hub is not guarded to protect employees from the rotating part. d) Appalachian Wood Products, Inc. 171 Appalachian Drive Clearfield, PA 16830 - On or about 04/06/2023 and at times prior thereto, employees were exposed to hazards associated with an unused portion of chains on machine #664 Mereen-Johnson Double End (serial #MJ1069) in the Assembly area. The unused portion of chains are along the bottom part of the machine and do not have any guarding in place.

29 CFR 1910.147(c)(6)(i)(C):Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected. a) Appalachian Wood Products, Inc. 171 Appalachian Drive Clearfield, PA 16830 - On or about 04/06/2023 and at times prior thereto, all authorized employees were not included in the periodic inspection of the lockout/tagout procedures. Employees perform servicing and maintenance on equipment that included, but was not limited to, machine #995 Accu-Systems miter saw (serial # 005-06-MMTJ-3-APPPRO).
