29 CFR 1910.95(k)(1): The employer shall train each employee who is exposed to noise at or above an 8-hour time weighted average (TWA) of 85 decibels (dBA) in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program. At the facility located at 3312 Lakeshore Drive, Sheboygan, WI 53081, the employer did not provide training to employees exposed to noise at or above an 8-hour TWA of 85 dBA in accordance with the requirements of 29 CFR 1910.95(k), exposing employees to the hazards associated with noise in the following instances: a) On or about September 23, 2021, an employee working in Bay 36 was exposed to continuous noise at 105.9% of the action level (85 dBA as an 8-hour TWA) or an 8-hour TWA of 90.4 dBA during the 578 minute sampling period. b) On or about September 23, 2021, an employee working in Bay 36 was exposed to continuous noise at 133.9% of the action level (85 dBA as an 8-hour TWA) or an 8-hour TWA of 92.1 dBA during the 564 minute sampling period.

29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. At the facility located at 3312 Lakeshore Drive, Sheboygan, WI 53081, the employer did not enter all information required to complete the employer's OSHA 300 form for 2021 in the following instances: a) On or about June 23, 2021, the employer did not ensure that case 2021-13 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. b) On or about July 1, 2021, the employer did not ensure that case 2021-14 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. c) On or about July 7, 2021, the employer did not ensure that case 2021-16 on the employer's OSHA 300 form for 2021 contained a complete description of the injury or illness and the parts of the body affected. d) On or about July 7, 2021, the employer did not ensure that case 2021-17 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. e) On or about July 20, 2021, the employer did not ensure that case 2021-18 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. f) On or about July 20, 2021, the employer did not ensure that case 2021-19 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. g) On or about July 23, 2021, the employer did not ensure that case 2021-20 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. h) On or about August 19, 2021, the employer did not ensure that case 2021-21 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill. i) On or about August 31, 2021, the employer did not ensure that case 2021-22 on the employer's OSHA 300 form for 2021 contained a complete description of the object/substance that directly injured or made the person ill.
