On or about September 3, 2021, Watry Industries, LLC failed to conduct lockout/tagout periodic inspections at least annually for the energy control procedures in Bay 36.

29 CFR  1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   On or about September 3, 2021, Watry Industries LLC. failed to provide lockout/tagout training authorized employees working in Bay 36 that were performing servicing and maintenance activities, such as, but not limited to, cleaning, blade changes on band saws, and breaking jams on the conveyor system for the Cutting Robot.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a.  On September 3, 2021, an employee was exposed to machine hazards associated with moving parts when they engaged in a blade change activity on the MBD Vertical Band Saw (Equipment # SWB008) in Bay 36.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to change saw blades.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  b.  On September 3, 2021, an employee was exposed to machine hazards associated with moving parts when they engaged in unjamming activities at the flip station for the Cutting Robot in Bay 36.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to reach their hands through the perimeter guarding to unjam stuck castings.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  c.  On September 3, 2021, employees were exposed to machine hazards associated with moving parts when they engaged in spilled Aluminum cleanup activities in-between the dies of the Tilt-Pour A (Equipment # CMH008) in Bay 36.  The employer did not ensure that employees implemented energy control application steps prior to allowing them put their hand between the dies while installing the burning bar.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

29 CFR  1910.212(a)(3)(ii):The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  On or about September 3, 2021, in Bay 36, the Vertical Band Saw  (SWB008) guard was adjusted to 5 1/2" above the highest part of the casting exposing employees to amputation hazards.  Watry Industries, LLC  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1426844, citation number 1, item number 1(b) and was affirmed as a final order on October 22, 2019, with respect to a workplace located at 3312 Lakeshore Drive, Sheboygan, WI 53081.

29 CFR  1910.22(a)(1):(1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  On or about September 3, 2021, the employer did not provide a clean walking-working surface in the vicinity of Tilt Pour Machine C in Bay 36 after a repaired hydraulic leak.
