Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) The Hyatt House at 17254 Five Points Square Lewes, DE 19958: On or about September 10, 2021, the employer failed to ensure employees had and used fall protection while working approximately 41 feet above grade, exposing employees to fall hazards.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) The Hyatt House at 17254 Five Points Square Lewes, DE 19958: On or about May 13, 2021, the employer failed to ensure employees used ladders that extended at least 3 feet above the upper landing to access the upper levels of a multi story building, exposing employees to fall hazards approximately 41 feet above grade.
