This was most recently demonstrated on or about March 30, 2023, and at times before, when employees were framing a wood building with the use of pneumatic nail guns and were not wearing protective eyewear.

29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: At a worksite located at 29884 St. Hwy 190, Chillicothe, MO: The employer failed to protect employees from exposure to fall hazards. This was most recently demonstrated on or about March 30, 2023, when a metal platform approximately 10 ft x 2 ft was placed on the forks of a rough terrain fork truck and utilized to lift framing materials to the roof section of the structure. The platform and materials onboard were lifted into position near the roof area, and an employee was standing on the platform passing materials to another employee. The platform could have shifted and tipped, resulting in the materials and the employee being ejected and falling approximately 14 feet to the ground below. The platform was not secured to the truck.

This was most recently demonstrated on or about March 30, 2023, and at times before, when employees were framing a wood building and were not provided any fall protection systems or wearing any personal fall protection. Employees were not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. 29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: At a worksite located at 29884 St. Hwy 190, Chillicothe, MO: The employer failed to protect employees from potential fall hazards. This was most recently demonstrated on or about March 30, 2023, and at times before, when employees were framing a wood building and were observed climbing the walls of the structure in lieu of using a ladder or man lift to access different levels on the building structure. The horizontal wall supports being used were spaced more than 19" apart from each other. Employees were not protected from falling by being provided a safe means of access or egress. The JMT Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1916.501(b)(2)(ii)), which was contained in OSHA inspection number 1540044, citation number 1, item number 4, and was affirmed as a final order on (January 10, 2022), with respect to a workplace located at 3823 N. Cobbler Road, Independence, MO 64058.
