29 CFR  1926.102(a)(1):Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At jobsite located at 8904 Greeley St. Lenexa, KS, employees engaged in residential framing activities using a circular saw were exposed to struck-by hazards in that eye protection was not utilized.

29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At jobsite located at 8904 Greeley St. Lenexa, KS, employees engaged in framing activities were exposed to electrical and burn hazards in that the flexible cord protective layer was pulling away from connector.
