29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. Employees were exposed to struck-by hazards. Employees engaged in framing activities with the use of pneumatic nail guns were exposed to struck-by hazards. Employees were not provided with eye protection.

29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes; Employees were exposed to fall hazards. Employees engaged in framing activities were exposed to fall hazards in that there was an opening on the floor approximately 9 3/4 feet above the lower level that was not protected with a type of guardrail system.
