29 CFR  1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects was immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with 29 CFR 1910.145 and removed from service until repaired in accordance with 29 CFR 1910.22(d), or replaced:  Building 6 Warehouse: Employer is exposing employees to hazardous conditions where the employees are permitted to use defective manually propelled ladder stands.  On and about 12/22/2022,  employees were exposed to fall hazards where they used defective manually propelled ladder stands which were not tagged as defective or removed from service.

29 CFR  1910.37(a)(3): The employer did not ensure that exit routes were free and unobstructed:  Building 6, Warehouse: The employer is exposing employees to hazardous conditions where the employees are permitted to work where exit routes are not free and unobstructed. On and about 12/22/2021, employees worked in the warehouse where an exit route was blocked by pallets of products stored on workplace floor exposing the employees to trip hazards.

29 CFR  1910.176(a): Where mechanical equipment was used, aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard:  Building 6, Warehouse and Dock area: Employer is exposing employees to struck-by and trip hazards where the employees are permitted to work where the aisles and passageways are blocked.  On and about 12/22/2021,  employees were exposed to struck-by and trip hazards where the aisles and the passageways were blocked by stored materials where forklifts and floor jacks were used.  Two instances of the violation were:                                                  a.  Warehouse area where the aisles were blocked                                                   b.  Dock area where prominently marked permanent aisles were blocked by pallets of products.

29 CFR  1910.176(b): Storage of material created a hazard:  Building 6, Warehouse: Employer is exposing employees to hazardous conditions where storage of products creates a hazard to the employees.   On and about 12/22/2021, employees worked where the vertical height of stored products on pallets was within 10 inches of the ventilation duct and the sprinkler systems limiting the space for material handling equipment such as, but not limited to, powered industrial trucks to move the  pallets of products.  The employees were exposed to struck-by and falling object hazards.

29 CFR  1910.176(e): Clearance signs to warn of clearance limits were not provided where mechanical handling equipment was used:   Building 6, Warehouse:  Employer is exposing employees to hazardous conditions where the employees operate material handling equipment to move pallets of products onto storage shelves without knowing the clearance limits for storage. On and  about 12/22/2022, employees were exposed to falling object hazards where clearance limit sign was not posted to warn the employees of the limit clearance.

On and about 12/22/2021, employees were exposed to electrical hazards where there were several electrical boxes which had knockouts missing.
