29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  a) Barington Court, Building 3, Mt. Holly NJ: The employer did not ensure that each employee who utilized a Pettibone Traverse All-Terrain lift  at the worksite had received training which included formal instruction, hands on and an evaluation with a written certification of completion, on or about 01/19/22.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  a- Barington Drive, Building 3, Mt. Holly NJ: On or about  01/19/22 a portable electric circular saw, in use by the workers, was observed being powered with a flexible cord that was not provided with strain relief as a result of having been previously damaged.

Holly NJ: On or about  01/19/22 portable electric equipment, in use by the workers, were observed being powered with flexible cords that were missing  their outer protective covering therefore exposing their inner wiring to damage.

This violation was most recently observed, on or about 01/19/22, at Barington Court, Building 3 in Mount Holly NJ:   a- Barington Court, Building 3, Mount Holly NJ: A worker was exposed to fall hazards, up to and in excess of 8 feet 8 inches to the ground and at times the cement surface below, when walking and working from the top plates of a commercial structure's wall frames without the use of fall protection.
