B) A) Bucyrus Railcar Repair, on or about January 20, 2022 and at times prior thereto, an electrical disconnect on the east side of the building in the break area was wired into a 240V box that did not have a face covering on the disconnect.

A) Bucyrus Railcar Repair, on or about January 21, 2022 and at times prior thereto, an electrical outlet box located on the east side of the facility next to the dock door was not secured from the hole in which it was installed. The outlet was not secured into place with screws to prevent it from falling out of the wall hole where it was installed.
