29 CFR 1910.305(g)(1)(i):Flexible cords and cables were not approved for conditions of use and location: A) Bucyrus Railcar Repair, on or about January 21, 2022 and at times prior thereto, Extension cords were running through puddles of water and in some cases submerged. The extension cords being used are not rated for water. The extension cords were utilized to power equipment including but not limited to handheld portable grinders.

29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition: A) Bucyrus Railcar Repair, on or about January 21, 2022 and at times prior thereto, the floor in the fabrication shop had water accumulations from snow melting from the railcars. The floor was not maintained in a dry condition. Condition created a potential slip/fall hazard. In addition, an electrical hazard was created because personnel were standing in the water while performing welding operations.
