29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  a) 10 E Lea Blvd Wilmington, Delaware 19802 (West bound): On or about February 9, 2022, the employer failed to ensure frequent and regular inspections of the jobsite and equipment were conducted by a person competent in work zone safety standards which resulted in several hazardous conditions which exposed employees to struck by/caught in hazards when they performed site preparation work for water main repair.

1926.200(g)(2)   All traffic control signs or devices used for protection of construction workers shall conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA, which are incorporated by reference. Temporary traffic barriers used did not conform to provide protection for the construction equipment and employees. Construction vehicles were observed exposed when the channeling devices did not fully enclose the work area according to Section 6C.08.
