29 CFR  1926.501(b)(6):Each employee on ramps, runways, and other walkways was not protected from falling 6 feet (1.8 m) or more to lower levels by guardrail systems:  On or about 2/9/2022, on the west end of the tunneling pit, employees used a 2 x 12 inch walkway that spanned an excavation that was approximately 9.7 feet deep for access.    Employees were exposed to the hazard of falling to the ground below in that the walkway did not have guardrails.

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed,  a grasping device was not provided to assist employees in mounting and dismounting the ladder.     On or about 2/9/2022, employees used a 24-foot long aluminum extension ladder to gain access to and exit from the tunneling pit.  The ladder was installed inside of the trench shield and did not extend at least 3 feet above the top of the shield exposing employees to a fall hazard.
