29 CFR  1910.303(e)(1)(i):  Electric equipment was used without the manufacturer's name, trademark, or other descriptive marking by which the organization responsible for the product could be identified:  Maintenance Shop:  On, or about, April 1, 2022, a wall mounted panelboard that energized water heaters, outlets and another panel was not provided with the manufacturer's information or other markings to indicate the manufacturer.  The panel board was provided with Square D circuit breakers.  The door was missing, which may have contained the labels for the enclosure.

29 CFR  1910.303(e)(1)(ii):  Markings were not provided on electrical equipment for voltage, current, wattage and other ratings as necessary:  Maintenance Shop:  On, or about, April 1, 2022, a wall mounted panelboard that energized water heaters, outlets and another panel was labeled with the voltage for the enclosure.  Based on the use of single pole, double pole and three pole breakers, the enclosure appeared to be 120/208 volts.  The door was missing, which may have contained the labels for the enclosure.

29 CFR  1910.332(b)(1):  Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:  Plant:  On, or about, April 1, 2022, Maintenance personnel had not been trained regarding what work practices and protective equipment was required to protect them against electric shock, arc flash and arc blast hazards while performing live electrical work, live trouble-shooting and verifying whether or not electrical equipment was properly de-energized.

29 CFR  1910.332(b)(2):  Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety:    Plant:  On, or about, April 1, 2022, an employee who did not meet the criteria of a "qualified person" performed tasks such as diagnostic testing and programming of machines and equipment while the equipment was live.  The employee had not received training regarding electrical circuits and equipment, could not identify the voltage of the electrical enclosures that were accessed, was not familiar with the components inside of the enclosures and did not understand the potential hazards associated with the equipment.  Abatement Note:  Under Notes 1 and 2 of the definition of "Qualified person" in 29 CFR 1910.399, an unqualified person can perform certain tasks if provided with proper training, has demonstrated skills for the task(s) an is under the direction of a qualified person.

29 CFR  1910.335(a)(1)(i):  Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:  Plant:  Personal protective equipment such as, but not limited to, voltage rated gloves, fire resistive clothing, face shields, fire resistive balaclavas, hard hats and hearing protection were not provided to employees who performed live electrical work, including trouble-shooting, or who verified that electrical equipment was properly de-energized.  In addition, non-fire resistive hair nets were worn by employees while working on or near live electrical equipment.

29 CFR  1910.305(b)(1)(i):  Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions:  Production Floor, Pallet Highway:  On, or about, April 1, 2022, conductors exiting the Wulftec control box were not secured in proper fittings, they appeared to be secured with electrical tape.
