29 CFR 1910.119(j)(4)(ii):  Inspection and testing did not follow recognized and generally accepted good engineering practices:  Engine Room: On, or about, October 24, 2023, a gasket failed on Compressor 102 due to an over-pressurization incident after a slug of liquid ammonia was sent to the compressor. A high liquid level cutout located on the sight column along with other controls were determined to be the most likely cause of the incident, which resulted in a release of anhydrous ammonia from the low side refrigeration system. Inspection, testing and maintenance (ITM) records from the company indicated that the last documented functional test of the high liquid level cutout was performed on December 22, 2020. Recognized and generally accepted good engineering practices such as those found in the International Institute of Ammonia Refrigeration (IIAR) ANSI/IIAR 6 Standard for Inspection, Testing, and Maintenance of Closed-Circuit Ammonia Refrigeration Systems require that functional tests be performed and documented on an annual basis.
