29 CFR 1904.32(a)(2): The employer did not create an annual summary of injuries and illnesses recorded on the OSHA log: a) C/P Blue North - On or about March 22, 2022, the employer did not create an annual summary of injuries and illnesses on an OSHA 300A form to summarize the recordable injuries and illnesses for calendar year 2020 to include the number of days work was lost or restricted, and the types of injuries and illnesses that occurred which should have been posted for employee notice in a highly visible area of the worksite during February 1 through April 30 of calendar year 2021.
