An employee performing rigging tasks was exposed to crushed-by hazards when positioned between an 8000 lb mold cast being lifted via gantry crane and an adjacent stationary mold cast.

On or about April 13, 2022, at the address of 120 Filbert Street, Elyria, Ohio 44035: The employer did not ensure an four legged alloy steel chain slings used to hoist 8000lbs mold casting(s) had legible identification marking identifying size, grade, capacity, and reach prior to use. Employees using the the sling in this condition were exposed to struck-by hazards in that information identifying use parameters were not available for employee reference.
