The hooks of the 2-legged chain sling were not fastened to the castings to prevent displacement. Employees performing lifting operations in this condition were exposed to struck-by hazards.

29 CFR 1910.243(a)(2)(iii)(a): Hand-held powered tools, such as, but not limited to, platen sanders, grinders with wheels 2 inches in diameter or less, disc sanders with discs 2 inches in diameter or less, routers, planers, laminate trimmers, nibblers, shears, saber, scroll, and jig saws with blade shanks a nominal one-fourth of an inch wide or less, were not equipped with either a positive "on-off" control, or other controls as described by paragraph (a)(2)(i) and (ii) of this section. A) Elyria Foundry LLC, on or about June 8, 2022, at the address of 745 Bullocks PKWY Lorain Ohio: The employer did not ensure that a horizontal grinder used to clean/deburr castings was used with a positive control that disengages power when the pressure is released. Employees using the hand tool in this condition were exposed to laceration hazards.
