At the residential framing job site, at 1548 Tan Tara Trail in Jacksonville Florida. On or about May 3, 2022: Two framing carpenter employees who accessed the roof from stepladders were exposed to a 9-foot fall hazard in that the roof access ladders did not extend 3 feet above the edge of the roof.

At the residential framing job site, at 1548 Tan Tara Trail in Jacksonville Florida, on the right side of the house. On or about May 3, 2022: A framing carpenter employees were exposed to a 9-foot fall hazard in that he stood on the top and top step of the Werner 10-foot stepladder to access the roof. b. At the residential framing job site, at 1548 Tan Tara Trail in Jacksonville Florida, on the back side of the house. On or about May 3, 2022: A framing carpenter employees were exposed to a 7-foot fall hazard in that he stood on the top and top step of the Werner 8-foot stepladder to access the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: On the roof, at 1548 Tan Tara Trail in Jacksonville, Florida: On or about May 3, 2022, the employer exposed employees to 9 feet to 15 feet fall hazards, in that fall protection was not utilized by employees while installing roof truss bracing and roof sheathing. Bustos Framing, LLC, a predecessor to L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1563539, citation number 1, item number 2 and was affirmed as a final order on January 3, 2022, with respect to a workplace located at 1893 Carter Landing Boulevard, Lot 90, Jacksonville, Florida. Luis Fernando Bustos-Guzman, a predecessor to Bustos Framing, LLC and L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1285206, citation number 1, item number 1 and was affirmed as a final order on March 21, 2018, with respect to a workplace located at 3040 Lucena Lane, Jacksonville, Florida.
