At 88 Newton Lane, Saint Augustine, Florida, on the second floor:  On or about May 18, 2022, the employer exposed employees  to fall hazards, in that,  employees were using a 8-foot A-framed ladder to access 10-foot roof. The ladder was not high enough to reach safe the work area. The employees had to stand on the top rung.

On a 6/12 pitched residential roof located at 88 Newton Lane, Saint Augustine, Florida: On or about May 18, 2022, the employer exposed four employees to fall hazards in approximately 21-feet, in that fall protection was not utilized by employees, while performing roofing work.  L & B Framing LLC fails to assure effective protections against falls as required by the standard, as demonstrated on the ceiling joists at a residence, located at  88 Newton Lane, Saint Augustine, Florida: On or about May 18, 2022, the employer exposed three framing employees to include himself to an approximately 21 foot fall hazards from the roof to the ground level while installing roof paper, in that they were not protected by a fall protection system.  Bustos Framing, LLC, a predecessor to L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1563539, citation number 1, item number 2 and was affirmed as a final order on January 3, 2022, with respect to a workplace located at 1893 Carter Landing Boulevard, Lot 90, Jacksonville, Florida.  Luis Fernando Bustos-Guzman, a predecessor to Bustos Framing, LLC and L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1285206, citation number 1, item number 1 and was affirmed as a final order on March 21, 2018, with respect to a workplace located at 3040 Lucena Lane, Jacksonville, Florida.
