On a new residential construction, at a temporary jobsite located 73 Holly Ridge way, in Saint Augustine, FL: On or about March  04, 2025, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees while operating pneumatic nail guns.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On a new residential construction, at a temporary jobsite located 73 Holly Ridge way, in Saint Augustine, FL: On or about March  04, 2025 the employer exposed employees to 10 feet to 11 feet fall hazards from the second floor level to the ground level, in that, fall protection system was not  effectively utilized by employees while doing roofing work.   L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1598850 citation number 2, item number 1 and was affirmed as a final order on August 29, 2022, with respect to a workplace located at 357 Brookgreen Way, Saint Augustine, Florida  Bustos Framing, LLC, a predecessor to L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1563539, citation number 1, item number 2 and was affirmed as a final order on January 3, 2022, with respect to a workplace located at 1893 Carter Landing Boulevard, Lot 90, Jacksonville, Florida.  Luis Fernando Bustos-Guzman, a predecessor to Bustos Framing, LLC and L & B Framing, LLC, was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1285206, citation number 1, item number 1 and was affirmed as a final order on March 21, 2018, with respect to a workplace located at 3040 Lucena Lane, Jacksonville, Florida.

On or about  March 04, 2025, at a residence located at  73 Holly Ridge Way, Saint Augustine FL,  the employer exposed employees to fall hazards, in that A frame ladders were being utilized for the purpose of make shift scaffolding for which the A frame ladders were  not designed .
