29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) 15 Zwill Ct Spring Valley NY 10977 raised on forklift : A Pettibone Model 944 Extendo rough terrain forklift was used to raise an employee who was positioned on plywood. The platform was not approved forklift man basket exposing employee to fall hazard. Condition observed on or about 05/13/2022.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 15 Zwill Ct Spring Valley NY 10977 residential structure roof: Employees were installing plywood while positioned on roof exposed to fall hazards of approximately 30ft to the ground below as a means of fall protection was not utilized. Condition noted on or about 5/13/2022.
