29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about May 17, 2022, at the jobsite, an employee was in use of an electrical extension cord with the outer cover of the cord pulled away from the plug. This exposed the employee to an electrical hazard.
