29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: Employees engaged in leading edge construction at the construction site at 187th and Maple St, Elkhorn, Nebraska were exposed to fall hazards in that they were working on a leading edge that was over 11 feet above the ground and they were not protected by any fall protection systems.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches, whichever is less, were not equipped with a stair rail system along each unprotected side or edge: Employees engaged in construction activities at 1187th and Maple St, Elkhorn, Nebraska, were exposed to fall hazards in that a stairway with 13 risers and a height over 6 feet did not have a stair rails along the unprotected edge.
