Diversified Coatings Inc dba Allegheny Coatings - On or about 5/27/22, an employee sustained a recordable injury while working in the Plating shop/barrel line. The injury was not recorded on the 300 log, nor was a 301 completed, within 7 calendar days of the injury. As of 6/7/2022, the OSHA 300 Log and OSHA 301 were not completed for the injury.

Diversified Coatings Inc dba Allegheny Coatings - On or about 5/27/2022 and at times prior thereto, the company did not report a work-related hospitalization caused by a chemical burn. The chemical burn happened on 5/27/2022 at 2:30 pm . The hospitalization was not reported to OSHA until 6/7/2022 at 11:33 am.
