29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a- 14, 16, 18, 20, 22, 24, & 26 Foundry Boulevard, Mount Holly NJ: On or about 07/12/22 a worker was exposed to fall hazards, in excess of 12 feet 8 inches to the ground surface below, when working/walking from an four pieces of OSB that were resting, unsecured on the elevated forks of a JCB Identification Number JCB5CFKGC02438024 all terrain fork truck. This piece of equipment was not specifically designed by the manufacturer to support a scaffold platform. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(f)(5): Scaffolds were moved horizontally while employees were on them although the scaffolds were not designed for such movement by a registered professional engineer: a- 14, 16, 18, 20, 22, 24, & 26 Foundry Boulevard, Mount Holly NJ: On or about 07/12/22 a worker was exposed to fall hazards, in excess of 12 feet 8 inches to the ground surface below, when working/walking from an four pieces of OSB that were resting, unsecured on the elevated forks of a JCB Identification Number JCB5CFKGC02438024 all terrain fork truck while the all terrain fork truck moved horizontally. This worker was not utilizing any fall protection at the time of the exposure. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
