29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about July 13, 2022, at the above addressed jobsite, employees performing residential carpentry operations, ten (10) feet five (5) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.  b. On or about July 13, 2022, at the above addressed jobsite, an employee performing residential carpentry operations, ten (10) feet six (6) inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  Armadores Tepa Inc. was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(13) which was contained in OSHA inspection number 1564443, citation number 1 item number 1 and was affirmed as a final order on February 14, 2022, with respect to a workplace located at 22893 Sara Spring Drive, Frankfort, IL. 60423.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a.  On or about July 13, 2022, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.  Armadores Tepa Inc. was previously cited for a violation of this Occupational Safety and Health Standard or equivalent 1926.503(b)(2), which was contained in OSHA inspection number 1564443, citation number 2 item number 1 and was affirmed as a final order on February 14, 2022, with respect to a workplace located at 22893 Sara Spring Drive, Frankfort, IL. 60423.
