29 CFR 1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On or about November 13, 2024, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)   a. On Or about November 13, 2024, at the above addressed jobsite, the portable ladder used to access the beam's surface did not extend at least three (3) feet above the upper landing surface, thereby exposing an employee to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. On or about November 13, 2024, at the above addressed jobsite, an employee performing residential carpentry operations, fourteen (14') feet six (6") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  b. On or about November 13, 2024, at the above addressed jobsite, an employee performing residential carpentry operations, twelve (12') feet two (2") inches above the ground, was not protected from falls, thereby exposing the employee to fall hazards.  Armadores Tepa Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1755949, citation number 1, item number 1and was affirmed as a final order on August 15, 2024, with respect to a workplace located at 11060-11064 Lizmore Ln., Orland Park, IL 60467.  Armadores Tepa Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1607482, citation number 1, item number 1, and was affirmed as a final order on October 25, 2022, with respect to a workplace located at Lot 31 Mayfield Dr. in Frankfort, Illinois.   Armadores Tepa Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1564443, citation number 1, item number 1, and was affirmed as a final order on February 14, 2022, with respect to a workplace located at 22893 Sara Spring Dr. in Frankfort, Illinois.
