(a) On or about July 20, 2022 and at times prior, for the employees exposed to fire related hazards while performing work activities in the telecommunication shop located at 702 North Industrial Way, Ely, Nevada.  Notice 1, Item 3, 29 CFR 1910.37(b)(2) as required by 29 CFR 1960.8(b): The employer failed to clearly mark each exit with a sign reading "Exit". The employer was cited for a violation of the same standard and same hazard on March 12, 2020, Inspection No. 1465262, date April 3, 2020 of final order, and means of abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) On or about July 20, 2022 and at times prior, for the employees exposed to electrical and/or fire hazards where a relocatable power tap (power strip) was used to power an appliance (microwave) in the fire dispatch area located at 702 North Industrial Way, Ely, Nevada.  (b) On or about July 20, 2022 and at times prior, for the employees exposed to electrical and/or fire hazards while preforming work activities where relocatable power taps (power strips) were series-connected in the fire protection cubical area located at 702 North Industrial Way, Ely, Nevada.  Notice 1, Item 2, 29 CFR 1910.303(b)(2) as required by 29 CFR 1960.8(b): The employer failed to use or install listed or labeled electrical equipment in accordance with the instructions included in the listing or labeling. The employer was cited for a violation of the same standard and same hazard on June 28, 2021, Inspection No. 1535400, date April 3, 2020 of final order, and means of abatement.
