29 CFR 1926.403(b)(2):Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a. SandCastle Theater Stage Area: A large industrial fan and two portable light stands used to ventilate and illuminate the work area were powered by a pigtail style splitter connected to two extension cords in series connected to a relocatable power tap plugged in to a wall outlet with a damaged faceplate. Employees were exposed to electrical shock and fire hazards. Smithbridge Guam, Inc. was previously cited for a violation of the equivalent Occupational Safety and Health Standard 29 CFR 1910.303(b)(2), which was contained in OSHA inspection number 1416961, citation number 1, item number 3 and was affirmed final order on 9/16/2019, with respect to a workplace located at 300 Chalan Padiron Haya in Yigo, Guam.
