29 CFR 1910.147(c)(7)(i):The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  On or about July 20, 2022, the employer did not ensure that machine operators who perform service and maintenance tasks on the  Mateer Model #1900SX Serial# 24883/ Arenco Filler #2  tube filler machine were adequately trained on the means and methods needed to effectively isolate and control hazardous energy sources. Employees were exposed to the potential for unexpected start-up , energization and/or release of stored energy which result in amputation and crushing hazards while cleaning the tube filler nozzle and blades.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about July 20, 2022,  an employee suffered an amputation injury when the employee was in process of cleaning the product  from the Mateer Model #1900SX Serial# 24883/ Arenco Filler #2  tube filler machine.  The employer did not ensure that employees implemented energy control application steps prior to allowing them to clean the nozzle , tube holder and blades on the  Arenco Filler #2 tube filler machine. The machine was not  de-energized prior to the employees engaging in this servicing and maintenance activity, exposing them to amputation and crushing hazards from the unexpected movement of the machine.   The employer failed to implement energy control application steps as the machine was not isolated from all energy sources [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, involving LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
