29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On or about March 14, 2023, employees were exposed to corrosive materials, specifically NOROX MEKP-900. There was no adequate eyewash station or safety shower in the vicinity of the finishing tanks to quickly drench the eyes or skin of the employees in the event of exposure to corrosive substances. Abatement documentation is required for this item in accordance with the requirements of 29CFR 1903.19(d).

29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about November 10, 2022 to January 25, 2023, the employer did not require the operator to deenergize and lock out the press prior to performing servicing and maintenance activities such as cleaning the clamps/knifes/blades and tube holder on the Axomatic #4 while the machine was running. Operators were exposed to amputation and crushing hazards at unguarded fold jaws, crimp jaws and flatten jaws. Alternate protective measures such as machine guarding were not provided. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Operators were exposed to amputation and crushing hazards at unguarded fold jaws, crimp jaws and flatten jaws. The employer did not develop, document, and require operators to utilize machine-specific lockout/tagout procedures for the control of hazardous energy sources.
