29 CFR 1926.20(b)(1): A safety and health program was not initiated and maintained to provide compliance with the general safety and health provisions of the standard: a) Jobsite located at 1307 Germantown Ave, Philadelphia, PA - On or about July 27, 2022, the employer did not initiate or maintain an accident prevention program to address the associated hazards. Employees conducting construction activities, such as transporting materials and using equipment such as an aerial lift were exposed to struck-by and fall hazards.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Jobsite: 1307 Germantown Ave., Philadelphia, PA - On or about July 27, 2022, the employer did not provide a training program for each employee in the recognition and avoidance of fall hazards. An employee was utilizing an aerial lift without the use of a Personal Fall Arrest System, thereby exposing the employee to fall hazards.
