29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) Roof of new single-family home; One or more methods of fall protection was not in place while employees performed work from a roof at heights ranging between approximately 11 feet, 2 inches to 21 feet, 10 inches above ground level.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Northeast side of single-family home;  A Werner MT-22 twenty-two foot aluminum multi-position ladder placed at ground level and used as an extension ladder to access the roof, was not of sufficient height to allow for the side rails to extend at least 3 feet above the landing surface, was not equipped with a grasping device(s), and was not effectively secured to prevent displacement.
