29 CFR 1926.1053(b)(1):  Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) North side of residential structure; A Werner brand 22-foot telescopic multi-position aluminum ladder placed at ground level and used as an extension ladder by employees to access the roof, was not adjusted to allow for the side rails to extend at least 3 feet above the landing surface, was not equipped with a grasping device(s) and was not effectively secured to prevent displacement.
