29 CFR 1910.133(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. The employer is failing to protect employees from eye and face hazards. This was most recently documented on or about August 30, 2022 at 6161 Abbott Dr. Omaha, Nebraska. The employer failed to ensure that employees were not exposed to eye and face hazards while operating the plasma cutter without providing a minimum shade of 6 to 8 for eye protection or shielding.

29 CFR 1910.133(a)(5):The employer did not ensure that each affected employee used equipment with filter lenses that had a shade number appropriate for the work being performed, for protection from injurious light radiation. The Employer is failing to protect employees from eye hazards. This was most recently documented on or about August 30, 2022 at a commercial plasma cutting bay located at 6161 Abbott Dr. Omaha, Nebraska. Employee was not using any filtered lens at a minimum shade of 8 for eye personal protective equipment while operating the plasma cutter, subjected to eye hazards.
