29 CFR  1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there is a hazard from flying objects:  (a)  Reyes Construction, LLC, 11705 Decatur St, Westminster, CO 80234: On or about 9/19/2022, the employer did not ensure that employees wore eye protection while using a pneumatic nail gun. This exposed employees to flying object and struck-by hazards.

29 CFR  1926.1053(b)(1): Where portable ladders were used to access an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Reyes Construction, LLC, 11705 Decatur St, Westminster, CO 80234: On or about 9/19/2022, the employer did not ensure that employees had a safe means to access the upper landing where work was being conducted. The ladder that was used to access the second floor balcony only extended two feet above the surface being accessed on the second floor balcony 18 feet above the ground. This exposed employees to fall hazards.
