29 CFR  1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  South corner of customer parking lot: Employees were working on form work in an unprotected excavation approximately 8.5 feet deep and were not protected from excavated soil piled less than 2 feet from the edge.  Employees were exposed to struck-by hazards due.

29 CFR  1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  South corner of customer parking lot: Employees were working on concrete form work in an unprotected excavation that was approximately 8.5 feet deep and were not protected from cave-ins.  Employees were exposed to crushed-by and struck-by hazards.

29 CFR  1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement:  South corner of customer parking lot: Inside the 8.5 foot deep excavation, there were two uncapped rebars.  Employees were exposed to impalement hazards.  The Giant Construction Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.701 (b), which was contained in OSHA inspection number 1538906, citation number 1, item number 1 and was affirmed as a final order on 11/09/21, with respect to a workplace located at Lot 7128-R3 in Songsong Hill Subdivision, Yigo, Guam, 96929.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).  South corner of customer parking lot: The extension ladder used by employees to access the 8.5-foot-deep excavation exceeded the allowable angle of the ladder. Employees were exposed to fall hazards.
