29 CFR  1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use. Reference: ANSI Z358.1 Emergency Eyewash and Shower Standard 5.4.4 The eyewash unit shall be located on the same level as the hazard and the path of travel shall be free of obstructions that may inhibit the immediate use of the equipment.  In Grill/Kitchen: Employees were handling corrosive chemical materials such as Monogram Bleach, Monogram High Temp Grill Cleaner, and Ecolab Grease Express High Temp Grill Cleaner where an emergency eye wash station in close proximity for flushing of the eyes was blocked by an open door covering the eye wash station. The employer had not kept the station free of blockage. Employees were exposed to severe eye burns or blindness.

29 CFR  1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were prominently displayed on the container or readily available in the work area throughout each work shift:  Bowling Alley Grill: On or about October 18, 2022, and times prior, employees used unlabeled secondary containers to hold chemicals used during disinfecting and cleaning of kitchen equipment and floors. The employer did not ensure containers had prominently displayed labels showing chemical name, associated hazards, and warnings. The employees were exposed to eye burns.

29 CFR  1910.1200(g)(1): Employers did not have a safety data sheet in the workplace for each hazardous chemical which they use:  Bowling Alley Grill: On or about October 18, 2022, and times prior, employees used Monogram Bleach, Monogram High Temp Grill Cleaner, Ecolab Grease Express High Temp Grill Cleaner, and Merrychef Cleaner without a safety data sheets being available onsite. The employer did not provide safety data sheets for each hazardous chemical used. The employees were potentially exposed to dermal and eye damage.

The employer did not conduct and document a hazard assessment including the person certifying the evaluation to determine personal protective needed for assigned tasks.  The employees were potentially exposed to dermal and upper respiratory irritation.

29 CFR  1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  Bowling Alley Rear Maintenance: An employee conducted ball return and multiple pin setter equipment component cleaning using chemicals without being provided training on the safety and health hazards associated with the chemicals at the time of the initial assignment.  The employee was exposed to eye injuries.
