On or about October 26, 2022 at jobsite located in189 Bloomfield Dr., Woodstock, IL 60098 employees used an extension ladder that did not extend three feet past the roof they were accessing, exposing the employee to a fall of approximately 10 feet. Quality Restoration & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1238256, citation 2, item 1, and was affirmed final order on February 5, 2018, with respect to a workplace located at 10645 Halligan Way, Mokena, IL 60448. Quality Restoration & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1345346, citation 1, item 1, and was affirmed final order on April 15, 2019, with respect to a workplace located at 304 Acorn St. North Aurora, IL 60542. Quality Restoration & Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1533897, citation 1, item 1, and was affirmed final order on December 15, 2021, with respect to a workplace located at 304 N. Lombard Ave., Oak Park, IL 60302.
