29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:    a) On or about July 19, 2023, employees were exposed to eye injury while operating nail guns without the benefit of eye protection.  Quality Restoration & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102 (a)(1) which was contained in OSHA inspection number 1633715, citation 1, item 2, and was affirmed as a final order on March 15, 2023, with respect to a worksite located at 189 Bloomfield Drive, Woodstock, Illinois.    No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) On or about July 19, 2023, employees were exposed to the hazard of falling approximately 20 feet while working without fall protection from pitched roof surfaces.  Quality Restoration & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1633715, citation 2, item 1, and was affirmed as a final order on March 15, 2023, with respect to a worksite located at 189 Bloomfield Drive, Woodstock, Illinois.   Quality Restoration & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1533897, citation 1, item 1, and was affirmed as a final order on December 15, 2021, with respect to a worksite located at 304 Lombard Avenue, Oak Park, Illinois.   Quality Restoration & Roofing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13) which was contained in OSHA inspection number 1345364, citation 1, item 1, and was affirmed as a final order on September 26, 2018, with respect to a worksite located at 304 Acorn Street, North Aurora, Illinois.
