29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) 600-A Kaiser Drive, Folcroft, PA - Facility: The Employer did not ensure that each powered industrial truck operator was competent to operate powered industrial trucks and electric pallet jacks such as, but not limited to Toyota Electric Forklift Truck 7FBEHU18, Toyota Electric Truck 8HBW23, 8FBE20U, etc. exposing employees to crush/struck-by hazards. Condition noted on or about November 14, 2022. Training shall consist of a combination of formal instruction, practical training, and evaluation of the operator s performance in the workplace.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees had not previously been trained about was introduced into their work area: a) 600-A Kaiser Drive, Folcroft, PA - Production: Employees were not provided information/training on the hazardous chemicals in their work areas. Employees are exposed to hazardous chemicals, including but not limited to the following: -Sodium Hydroxide, Sodium Hypochlorite, etc. contained in Quorum Yellow LP -Phosphoric Acid, Citric Acid, etc. contained in Quorum Red II
