29 CFR 1910.22(a)(2): The floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided: a) Facility located at 713 North 10th Street, Philadelphia, PA - On or about October 21, 2024, the employer failed to ensure that the floor in the noodle processing room was properly maintained and free of hazards, including standing water, thereby exposing employees to slip and fall hazards. b) Facility located at 713 North 10th Street, Philadelphia, PA - On or about October 21, 2024, the employer failed to ensure that the floor in the tofu processing room was properly maintained and free of hazards, including standing water, thereby exposing employees to slip and fall hazards.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Facility located at 713 North 10th Street, Philadelphia, PA - On or about October 21, 2024, the employer did not ensure the cutting and sealing parts of the Dixie Vac rice noodle packaging machine were properly guarded while the machine was in use, exposing employees to amputation hazards. b) Facility located at 713 North 10th Street, Philadelphia, PA - On or about October 21, 2024, the employer did not ensure the tofu press was properly guarded while the machine was in use, exposing employees to amputation and crush injuries.
