If such modifications or changes are made, the capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. In no case shall the original safety factor of the equipment be reduced.   At 4501 Bluestem in Maize, Kansas; employees engaged in residential framing activities were exposed to fall hazards in that the employees were lifted in a wooden job-made basket that was not approved by the manufacturer.

The ladder used to access the working levels of the structure did not extend 3 feet above the landing and was not secured.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   At the jobsite located at 4501 Bluestem in Maize, Kansas;  Employees engaged in residential framing activities, walking the top plates, were exposed to fall hazards of approximately 16 feet to the exterior ground below in that fall protection was not utilized.  Brent Lawrenz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1599995, citation number 2, item number 1 and was affirmed as a final order on 09-02-2022, with respect to a workplace located at 3138 Judith Ave. in Wichita, Kansas.  Brent Lawrenz was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1509835, citation number 2, item number 1 and was affirmed as a final order on 05-25-2021, with respect to a workplace located at 5102 26th St., WICHITA, KS 67205.
