29 CFR 1910.252(b)(4)(iv): Where a welder must enter a confined space through a manhole or other small opening, means shall be provided for quickly removing him in case of emergency. When safety belts and lifelines are used for this purpose they shall be so attached to the welder's body that his body cannot be jammed in a small exit opening. An attendant with a preplanned rescue procedure shall be stationed outside to observe the welder at all times and be capable of putting rescue operations into effect: Between November 1 and November 28 of 2022, at a facility located at 2121 Cleveland Avenue in Marinette, Wisconsin; employees entered confined spaces through small manway openings to conduct welding operations but were not provided with means for quickly removing them in case of emergency. Welder/Fabricator employees in Center Bay entered vessels through manways ranging from approximately 16" to 18" to grind/buff welded seams inside of vessels without wearing any retrieval or lifeline equipment.

29 CFR 1910.252(c)(4)(i): All welding and cutting operations carried on in confined spaces shall be adequately ventilated to prevent the accumulation of toxic materials or possible oxygen deficiency. This applies not only to the welder but also to helpers and other personnel in the immediate vicinity. All air replacing that withdrawn shall be clean and respirable: On February 1, 2023, at a facility located at 2121 Cleveland Avenue in Marinette, Wisconsin; employees conducting welding operations in confined spaces were not provided with ventilation to prevent the accumulation of toxic materials or possible oxygen deficiency. A Welder/Fabricator employee in Center Bay was MIG (metal inert gas) arc welding steel systems (fixtures) on the inside of a horizontal cylindrical steel vessel without the use of ventilation to prevent the accumulation of toxic metal fumes, gaseous byproducts of welding and/or oxygen displacing welding gases such as argon and carbon dioxide.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions: On February 1, 2023, at a facility located at 2121 Cleveland Avenue in Marinette, Wisconsin; a Welder/Fabricator employee in Center Bay was MIG (metal inert gas) arc welding while the insulation on their grounding cable was damaged which exposed the copper conductors within. The damaged cable was not replaced.
