29 CFR 1926.453(b)(2)(v):A body belt was not worn and a lanyard was not attached to the boom or basket while working from an aerial lift: a. On a temporary job site located at the Clubhouse, 4101 San Pablo Parkway, Jacksonville, Florida 32224; On or about January 21, 2023, the employer exposed two employees to fall hazards, in that safety harnesses with lanyards were not utilized by the employees while installing Hardiboard siding. The employees were exposed fall hazards ranging up to 15 feet high.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On a 3:12 pitched roof of a temporary residential worksite, at the Clubhouse, 4101 San Pablo Parkway, Jacksonville, Florida 32224: On or about January 31, 2023, the employer exposed two employees to fall hazards ranging from 10 to 15 feet, in that fall protection was not utilized by employees while installing Hardi-board siding. M.G. Residential Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1559113, citation number 01, item number 001 and was affirmed as a final order on December 15, 2021, with respect to a workplace located at 2023 Dutton Island Oaks Way, Atlantic Beach, Florida 32233.
