29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a.  At Lot 390 in Weathersford Place, Yulee, Florida, on or about October 17, 2023, the employer exposed employees to eye injury hazards from flying debris, in that an employee sheathing the roof with a pneumatic nail-gun was not protected by safety glasses.

29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person:  a.  At Lot 390 in Weathersford Pl., Yulee, Florida, on or about October 19, 2023, the employer failed to protect employees from fall hazards, in that, adequate anchor points were not used by employees doing framing work.

29 CFR  1926.503(a)(2)(ii):The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  a.  At Lot 390 in Weathersford Pl., Yulee, Florida, on or about October 19, 2023, the employer exposed employees to an 11 feet fall hazard, in that employees performing roof framing work at 6 feet or more above ground were not properly trained in the use of personal fall arrest systems.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    Employees performing roof truss installation framing work are not protected from fall hazards by the use of a fall protection system.  The employer fails to assure effective protections against falls as required by the standard, as demonstrated on or about October 19, 2023, at Lot 390 in Weathersford Place, Yulee, Florida:  The employer exposed employees to an 11-feet fall hazard, in that they were not protected by a fall protection system while doing framing work on roof trusses.  The employer, M.G. Residential Carpentry Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR-1926.501(b)(13), which was contained in OSHA inspection number 1669557, citation number 2, item number 1, and was affirmed as a final order on July 25, 2023, with respect to a workplace located at 5405 Moret Dr. East, Jacksonville, FL 32210.  The employer, M.G. Residential Carpentry Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR-1926.501(b)(1), which was contained in OSHA inspection number 1647704, citation number 2, item number 1, and was affirmed as a final order on March 20, 2023, with respect to a workplace located at 4101 San Pablo Parkway, Jacksonville, FL 32224.  The employer, M.G. Residential Carpentry Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR-1926.501(b)(13), which was contained in OSHA inspection number 1559113, citation number 1, item number 1, and was affirmed as a final order on December 15, 2021, with respect to a workplace located at 2023 Dutton Island Oaks Way, Atlantic Beach, FL 32233.
