29 CFR  1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end:  a) Worksite - On or about February 7, 2023 and at times prior thereto, an employee was working from a ladder jack scaffold approximately 18 feet above the ground. The platform extended more than 12 inches on both ends of the support brackets. The platform tipped causing the employee to fall to the ground.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a) Worksite - On or about February 7, 2023 and at times prior thereto, an employee was preparing to install roofing material from the platform of a ladder jack scaffold approximately 18-feet above the ground below.  The employee did not have adequate access to the ladder jack scaffold platform.
